    Due to overwhelming response, the survey deadline is extended to Monday afternoon. 

    Requirements:

    - 18 years and older

    - Full time employee (40 hours)

    - Employed in the private sector

    - Receive employer provided healthcare benefits - any aspect of health benefits

    This survey closes on Wednesday February 13th.
